Information Sheet on Background Checks for Education Students
Criminal background checks will be conducted for all undergraduate and graduate students admitted to UW-Madison School of Education programs that require field placements (e.g., practicum, student teaching, or clinical field experience).
Students accepting admission to Counseling Psychology must print, complete, and submit the Academic Background Information / Disclosure Authorization and Waiver Form to the Academic Services Coordinator. This form is available online on the Counseling Psychology website – under “MS Program>Resources for Current MS Students” and “PhD Program> Resources for Current PhD Students,” respectively. Submit this form to the Department of Counseling Psychology, Room 335 Education Building, 1000 Bascom Mall, Madison, WI, 53706-1326.
Students must then initiate a criminal background check through Certified Background, Inc. The cost for the background check is paid for by the Department of Counseling Psychology. You will have access to the background check results for yourself and to share with other agencies and institutions. The results will also be available to the director of the program and administrators connected to admissions and student affairs in the Department of Counseling Psychology. For each student, the background investigation will include a full residency check, a background review for every state of residency, and a check against additional offender databases.
Assignment to your practicum site will not be finalized until we have received the verification that your criminal background check has been run by Certified Background.com. Go to the Certified Background website at www.certifiedbackground.com, click on "Students," and enter the "package code” for the Department of Counseling Psychology. That will be emailed separately with additional instructions.
After you initiate the background check, you will receive an email stating that the background check will be started. When the background check is completed (between 2 days and 1 week), you will receive another email stating that the background check is done. You will also have an opportunity to view and print the results. We recommend that you print your results, since CertifiedBackground.com will not send you a paper copy, nor will the Department of Counseling Psychology. You do not need to bring the printed copy in to department, but we recommend that you keep it with your transcripts and other important papers. If any problems are found on the background check, you will be asked to send an email or letter to the Chair of the Counseling Psychology Department describing the case and its disposition. If s/he feels the need to discuss the case with you further, s/he will contact you.
